Welcome to BALLARD PRE-K! Our program offers a supportive environment for children so they can learn and have fun. The classroom community allows children to feel safe while instilling a sense of belonging in each child. We use the Creative Curriculum for a holistic approach and work on different themes each week. Together, the Social/Emotional, Physical, Cognitive and Language skills learned as a preschooler will help children in their overall development. This opportunity will prepare them for kindergarten, giving them the necessary experience for a great start to their scholastic career.

Additional Medical Forms - If Needed
If your child has known allergies, asthma or will need to be given medication, the following forms are required. Please bring these in along with EpiPens, inhalers or other medication your child requires. Be sure to review our Family Handbook for complete information on our Medication Policy.
This form is for any medication, aside from sun screen or hand sanitizer, you require us to give your child. This includes medication we keep on hand such as inhalers or EpiPens or daily medication. For occasional day-time medication delivery, bring this form with the medication.
For known allergies or intolerance to foods, etc. you will need your child's doctor to assist in completing the report.
Individual Plan of Care
Please fill out this form for children with specialized behavioral or medical needs. Each individual plan is a collaboration between the parent/guardian and our child care staff to best serve your child.
This form lists three levels of severity for us to watch for if your child has asthma and the course of action we need to take.
